The evaluative process is a critical part of the internship. This form gives you evaluative feedback on your performance, identifies areas to strive for improvement, clarifies your current strengths and weaknesses as related to the pursuit of a career, lets you assess the site and the quality of supervision you received, and helps your faculty supervisor determine a satisfactory/unsatisfactory grade. You and your site supervisor complete your evaluations independently, then discuss them jointly. Return this form to your faculty supervisor by the date specified on the internship contract.
